Blessed Sacrament extends a warm welcome to everyone who comes. As a faith community, we urge parishioners to register with the parish office.  This ensures we provide you and your family with the proper catechetical, pastoral, and sacramental care.

Please take a few moments to complete this electronic registration form (where applicable).  Please include ALL adults and children (under 21) family members that reside at home. Paper copies of the registration form are also available at the parish office.

Note:  Certificates of eligibility to sponsor at Baptism or Confirmation will ONLY be provided to registered parishioners who are active and practicing Catholics.  Only registered parishioners can be enrolled in our religious education program.  Kindly notify us if you move out of the parish.
